Medical History Form

To speed up your check-in process, please print and complete the medical history form and bring it with you to your appointment.

Appointments & Cancellations

Please contact the practice to book an appointment at a time convenient for you. If for any reason you must cancel your appointment, 24 hours notice is required or a cancellation fee may apply.

Payment

Payment is required at each appointment. HICAPS is available for direct billing of your health insurer and cash, credit card, and EFTPOS facilities are available.

Dental Centre Maroochydore is located at 23 Beach Road, Maroochydore – near the traffic lights on the corner of Beach Road and Duporth Avenue. Parking is available behind the practice, by going down the driveway to the left of our building. There is also 2hr parallel parking out the front on Beach Road.
